What is 'Homocysteine' and why should I test it?
Homocysteine is a toxic amino acid byproduct that indicates how efficiently your body processes B vitamins and manages cellular inflammation. High levels are a direct marker for increased risk of stroke, heart disease, and dementia as estrogen levels decline.

A level of 15 is a red alert. It means your body is failing to protect your heart and brain. It means your B vitamin status is likely abysmal, regardless of what your serum B12 levels say.
What to tell your doctor
Do not ask for a general wellness check. Be specific. Tell your doctor: I want a serum homocysteine test to assess my cardiovascular and neurodegenerative risk. Do not let them talk you out of it.
If they say it is unnecessary, tell them: Given my declining estrogen levels, I am concerned about my methylation efficiency and its impact on my long-term brain health. I want to establish a baseline now.
Ask for a full B-vitamin panel alongside it. Specifically, ask for Serum B12 and Folate. If your B12 is at the low end of normal, it is likely contributing to high homocysteine levels.
If your homocysteine comes back high, do not accept a prescription for a statin as the only solution. Statins do not lower homocysteine. You need to address the metabolic breakdown, not just the resulting cholesterol.
Use the phrase clinical risk stratification. This shows you understand the severity of the marker. It forces the doctor to treat the request with more weight. You are looking for a specific metric of internal health.
If your doctor refuses, find a new doctor. This test is inexpensive and standard. Refusal to test a known marker for stroke and dementia risk is a failure of clinical care.
What is a waste of time
Standard multivitamins are a waste. Most contain folic acid, which is the synthetic form of folate. Many women have a genetic mutation that makes it impossible to process folic acid. It actually blocks your receptors.
Menopause teas and herbal blends will not lower your homocysteine. They do not contain the concentrated B vitamins required to jumpstart the methylation cycle. They are marketing fluff designed to separate you from your money.
Detox foot pads and liver cleanses are scams. Your liver detoxifies through the methylation cycle. You cannot fix that cycle with a sticky pad on your foot or a juice fast. You fix it with biochemistry.
General exercise programs without nutritional support are insufficient. While exercise is good, it cannot overcome a fundamental B-vitamin deficiency or a genetic methylation block. You cannot out-walk a toxic amino acid buildup.
High-dose generic B-complex supplements can be problematic. If they contain the wrong forms of the vitamins, they can cause more harm than good. You must be specific about the chemical forms you ingest.

Pyridoxal-5-Phosphate (P5P) is the active form of Vitamin B6. It is a critical cofactor in the transsulfuration pathway, which is the secondary way your body clears homocysteine. Standard B6 is often not enough.
Trimethylglycine (TMG), also known as Betaine, is a powerful methyl donor. It provides an alternative pathway for the body to convert homocysteine back into methionine. This is often the fastest way to drop high levels.
Magnesium is essential for over 300 enzymatic reactions, including those in the methylation cycle. Use magnesium glycinate or magnesium malate to support the cellular energy required for these chemical conversions.
Consistency is mandatory. Methylation is a 24/7 process. You cannot take your supplements once a week and expect a result. You must maintain a steady state of these nutrients to keep the cycle running.
Re-test your levels every three months after starting a protocol. You want to see that number moving down toward 7. If it is not moving, you may need higher doses or a different combination of methyl donors.
What you can do right now

Eliminate processed foods containing enriched flour. These are loaded with synthetic folic acid, which can clog your methylation pathways if you have common genetic variants. Stick to whole, single-ingredient foods.
